BeefJerky, there are a bunch of ways you can get teleported. The teleport-swap, for instance, happens when some other creature gets teleported...

Yes, this is due to problems we're having with the video playback on the iPad. So, if you disable video for now, you will have no other problems....

wobblez, so far there doesn't seem to be a random teleportation bug. We don't know if he was zapped by an ogre for what. Can't reproduce = no bug....
