I believe if you kill enemies within the outer circle, it increases the multiplier. Not 100% sure though...

Yes. Fairly close to your core (like crossing over your ship but missing the core). Try watching your graze counter to get a feel for it.
I agree. I think it would be a shame to reduce the intensity of what's on screen. The extreme over the top mayhem is part of the fun for me.
